Job Title: District Nurse Specialist

We are seeking a highly skilled and motivated District Nurse Specialist to join our team at Transformation Unit. As a key member of our community nursing team, you will be responsible for providing high-quality patient care and contributing to the development of our services.

About the Role

This is an exciting opportunity to work in a dynamic and patient-focused environment, providing care to patients in their own homes. You will be working closely with a multidisciplinary team to deliver care that is tailored to the individual needs of our patients.

Key Responsibilities
  • Provide high-quality patient care, including assessment, planning, implementation, and evaluation of care.
  • Work collaboratively with other healthcare professionals to deliver care that is coordinated and effective.
  • Contribute to the development of our services, including the design and implementation of new care pathways.
  • Participate in quality improvement initiatives to ensure that our services are safe, effective, and patient-centered.
  • Provide education and support to patients, carers, and other healthcare professionals to promote health and wellbeing.
Requirements
  • Registered General Nurse, Adult Branch and current NMC Registration.
  • Experience of working in a community care setting, preferably with a focus on district nursing.
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
  • Commitment to ongoing professional development and continuing education.
