Key Responsibilities:
  • Coordinate care delivery to ensure seamless transitions and continuity.

  • Work collaboratively with the multidisciplinary team to ensure effective care delivery.

Requirements:
  • RGN or RMN qualified with a valid NMC pin.
  • At least 6 months' experience in a similar role.
  • Excellent leadership and communication skills.
  • Able to work flexibly, including days, nights, and weekends.
