Community Psychiatric Nurse Specialist

We are seeking a dedicated and compassionate Registered Mental Health Nurse to join our team at Annwylfan Ward, a 16-bedded Dementia Assessment Unit in Ysbyty Ystrad Fawr Hospital. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with dementia and their families.
Our goal is to deliver person-centered care, promoting independence, dignity, and well-being. You will have the opportunity to work closely with colleagues from various disciplines, including doctors, occupational therapists, and social workers, to develop and implement individualized care plans.
This role offers a unique chance to develop your skills and expertise in dementia care, with access to the Journey of Excellence (JoE) professional development programme. You will also have the opportunity to work in a dynamic and supportive environment, with flexible working hours considered to meet service needs.
About the RoleAs a Registered Mental Health Nurse, you will be responsible for assessing, planning, and delivering care to individuals with dementia. You will work closely with patients and their families to understand their needs, preferences, and goals, and develop care plans that promote their physical, emotional, and social well-being.
You will also be involved in the development and implementation of care pathways, policies, and procedures, ensuring that care is delivered in accordance with best practice and national guidelines.
Requirements- Registered Mental Health Nurse with a valid NMC registration
- Experience in dementia care, preferably in a hospital or community setting
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Flexibility to work varied shifts, including evenings and weekends
