Community Psychiatric Nurse Dementia Specialist

We are seeking a highly skilled and dedicated Community Psychiatric Nurse to join our Dementia Assessment Unit team at Ysbyty Ystrad Fawr Hospital. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals living with dementia.
Key Responsibilities- Assess and develop care plans for individuals with dementia, working closely with colleagues and other healthcare professionals.
- Provide expert care and support to individuals with dementia, promoting independence, dignity, and well-being.
- Collaborate with the multidisciplinary team to ensure seamless care delivery and effective communication.
- Participate in education and training programs to enhance knowledge and skills in dementia care.
