Community Psychiatric Nurse Specialist

We are seeking a dedicated and compassionate Registered Mental Health Nurse to join our team at Annwylfan Ward, a 16-bedded Dementia Assessment Unit based in Ysbyty Ystrad Fawr Hospital. Our goal is to provide exceptional care and support to individuals with dementia, promoting independence, maintaining contact with loved ones, and enhancing their quality of life.
This role offers a unique opportunity to access the Journey of Excellence (JoE) professional development programme, aligned with the learning and development opportunities within the Health Board. Flexible working hours will be considered to meet service needs.
Main ResponsibilitiesAs a key member of our team, you will be responsible for:
- Providing high-quality care and support to individuals with dementia
- Collaborating with multidisciplinary teams to develop and implement individualized care plans
- Conducting assessments and evaluations to inform care decisions
- Participating in education and training opportunities to enhance skills and knowledge

Person SpecificationWe are looking for a candidate who:

Applicants should have:
- Knowledge of basic dementias and extensive knowledge of dementia
- Experience in mental health, with a focus on older adult mental health
