Job Summary

The Central and North West London NHS Foundation Trust is seeking a skilled Peripatetic Recovery Specialist to join our multidisciplinary team. As a key member of our team, you will provide specialist services to individuals struggling with substance misuse issues, with a focus on harm minimization and community-based treatment.

Main Responsibilities
  • Conduct continuous assessments of clients' psychological, emotional, social, and rehabilitative needs, developing care plans and evaluating their effectiveness.
  • Provide case management and direct clinical care to a defined number of clients, including assessment, planning, and implementation of specialist care.
  • Work collaboratively with clients, families, and local communities to develop and implement personalized treatment plans.
  • Participate in multidisciplinary team meetings to discuss client progress and develop strategies for improvement.
  • Stay up-to-date with the latest research, best practices, and national and local pathways to access substance misuse support.
Requirements
  • Education to diploma level or NVQ level 4, or registered nurse or other registered healthcare professional.
  • Post-registration experience, with a focus on substance misuse services.
  • Practical experience in providing high-quality care, conducting risk assessments, and managing work under pressure.
  • Ability to work autonomously, using own initiative, and as part of a multidisciplinary team.
  • Excellent communication, organizational, and problem-solving skills.
Desirable Qualifications and Experience
  • Harm reduction training, motivational interviewing, and relapse management training.
  • Post-registration substance misuse qualifications, degree, or equivalent.
  • Previous experience within an NHS or local authority setting, or working in a prison setting.
  • Data processing and analysis skills, audit experience, and leadership skills.
