Peripatetic Recovery Specialist

We are seeking a skilled and compassionate Peripatetic Recovery Practitioner to join our multidisciplinary team at Central and North West London NHS Foundation Trust. As a key member of our team, you will provide high-quality care and support to individuals experiencing homelessness and substance misuse issues in Milton Keynes.
Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans for clients, taking into account their unique needs and circumstances.
- Provide direct clinical care and support to a defined caseload of clients, including those with low to medium complexity needs.
- Work collaboratively with other healthcare professionals to ensure seamless care coordination and effective treatment outcomes.
- Develop and maintain strong relationships with local community partners and stakeholders to enhance service delivery and access to resources.
- Stay up-to-date with best practices and evidence-based interventions in substance misuse treatment and homelessness services.
We are looking for an individual with:
- Relevant qualifications and experience in a healthcare or social care setting.
- Strong assessment and care planning skills, with the ability to work autonomously and as part of a team.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
- Knowledge of substance misuse treatment and homelessness services, including harm reduction approaches and motivational interviewing.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
