Peripatetic Recovery Specialist

We are seeking a skilled and compassionate Peripatetic Recovery Practitioner to join our multidisciplinary team at Central and North West London NHS Foundation Trust. As a key member of our team, you will provide high-quality care and support to individuals experiencing homelessness and substance misuse issues in Milton Keynes.
Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans for clients, taking into account their unique needs and circumstances.
- Provide direct clinical care and support to a defined caseload of clients, including those with low to medium complexity needs.
- Work collaboratively with other healthcare professionals to ensure seamless care coordination and effective service delivery.
- Develop and maintain strong relationships with local community partners and stakeholders to enhance service access and engagement.
- Contribute to the development and implementation of service policies and procedures, ensuring alignment with Trust standards and best practices.
We are looking for an individual with:
- Relevant qualifications and experience in substance misuse, mental health, or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse client groups and stakeholders.
- Strong assessment and care planning skills, with the ability to develop and implement effective interventions.
- Ability to work autonomously and as part of a team, with a flexible approach to work and a commitment to continuous professional development.
- Knowledge of local and national pathways to access substance misuse support, with a commitment to harm minimization and recovery-oriented practice.
