Peripatetic Recovery Specialist

About the Role
We are seeking a skilled Peripatetic Recovery Practitioner to join our multidisciplinary team at Central and North West London NHS Foundation Trust. The successful candidate will work with clients who are homeless or at risk of homelessness, providing specialist care and support to help them achieve their recovery goals.
Key Responsibilities
- Conduct assessments and develop care plans for clients, taking into account their psychological, emotional, and social needs.
- Provide direct clinical care to a defined caseload of clients, including assessment, planning, and implementation of care.
- Work collaboratively with other healthcare professionals to ensure a comprehensive and coordinated approach to client care.
- Participate in multidisciplinary team meetings to discuss client progress and develop strategies for improving outcomes.
Requirements
- Registered Nurse or other registered healthcare professional with experience of working in substance misuse services.
- Post-registration experience of providing high-quality care to clients with complex needs.
- Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
- Knowledge of mental health and substance misuse services, including harm reduction and motivational interviewing.
