About the Role

We are seeking a skilled and compassionate Community Support Worker to join our team at Cornwall Partnership NHS Foundation Trust. As a Community Support Worker, you will play a vital role in supporting individuals to regain their independence and confidence, enabling them to live fulfilling lives in their own homes.

Key Responsibilities
  • Deliver packages of care to patients in their own homes, including residential homes and nursing homes
  • Work collaboratively with a range of services to prevent hospital admission and support discharge from hospital
  • Provide a versatile and responsive service to ensure opportunities for patients' reablement are optimised within the community setting
  • Be flexible in working patterns to meet the needs of the service and work without direct supervision
Requirements
  • NVQ Level 3 qualification in care or equivalent experience
  • Evidence of experience in a care setting
  • Valid UK driver's license
