About the Role

We are seeking a skilled and compassionate Community Support Worker to join our Home First Team in Kerrier. As a key member of our team, you will play a vital role in delivering reablement services to patients in their own homes, promoting independence and supporting them to regain confidence.

The successful candidate will have a strong background in care and support, with excellent communication skills and the ability to work flexibly to meet the needs of our service. You will be part of a dedicated team and will receive bespoke training to support your development in this role.

Key Responsibilities
  • Deliver packages of care to patients in their own homes, including residential homes and nursing homes
  • Work collaboratively with other services to prevent hospital admission and support discharge from hospital
  • Provide flexible and responsive support to meet the needs of our service
  • Develop and maintain effective relationships with patients, families, and other stakeholders
Requirements
  • NVQ Level 3 qualification in care or equivalent experience
  • Evidence of experience in a care setting
  • Valid UK driver's license
  • Good communication skills and ability to work flexibly
