We are seeking a skilled and compassionate Community Support Worker to join our Home First Team in Kerrier. As a Community Support Worker, you will play a vital role in delivering reablement services to patients in their own homes, promoting independence and supporting them to regain confidence.
Main Responsibilities
The successful candidate will be responsible for providing a versatile and responsive service to ensure that opportunities for patients' reablement are optimised within the community setting. This will involve delivering packages of care to patients in their own homes, as well as in Residential homes and nursing homes.
Key Skills and Qualities
- Flexible and adaptable approach to working in a dynamic environment
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid UK driving licence and access to a vehicle
