Community Psychiatric Nurse

About the Role
We are seeking a dynamic and experienced Mental Health Nurse to join our Community Mental Health Team as a Care Coordinator. The successful candidate will be responsible for coordinating care needs, planning, and implementing evidence-based care to a defined group of service users and their carers within a defined catchment area/locality.
Key Responsibilities
- Act as the named care co-ordinator and manage the coordination of care needs, planning, and implementation of evidence-based care to a defined group of service users and their carers, within a defined catchment area/locality.
- Provide professional advice and support to other agencies and members of the multidisciplinary team, and act up for Team Lead in their absence as appropriate.
- Act as a role model and resource for junior/less experienced staff.
Requirements
- 1st level NMC Registered Mental Health Nurse (Degree/Diploma equivalent)
- Certificate in a psychotherapeutic intervention solution-focused therapy, Basic CBT, BFT, or counselling etc.
- Knowledge of Community Mental Health Team and Recovery principles
- Excellent working knowledge of the practices and principles of the Care Programme Approach
