About the Role

We are seeking a Community Care Specialist to extend our multidisciplinary teams across East Ayrshire communities. This role involves working alongside partner agencies to ensure everyone receives preventative care that maximizes independence.

Key Responsibilities
  • Protect and promote the welfare of Adults at Risk through statutory powers addressing complex issues.
  • Undertake assessments of need, formulate, and implement care plans according to the National Self-Directed Support Framework, ensuring adults remain in their homes with the necessary supports.
  • Collaborate with external agencies and partner organizations for positive outcomes, promoting the delivery and integration of Health & Social Care Partnership Plan objectives.
Work Arrangements

This full-time permanent position is based across Locality Health & Care. The workstyle is flexible, with 50% -79% of time spent in an office location or multiple work locations. You will work from your administrative base or another agreed office location between 2.5 to 4 days per week, occasionally working from home.

Compensation

The annual salary range is £41,000 - £44,000, including a 5% market forces payment.
