Care Coordinator

We are seeking a highly skilled and dedicated Scheduler/Care Co-ordinator to join our team at Home Instead. As a key member of our office, you will play a pivotal role in planning and building our schedules to ensure that our clients receive the highest quality care from our Care Professionals.
Key Responsibilities- Plan and coordinate schedules to ensure seamless delivery of care services
- Organize rotas and ensure Care Professionals have advance notice of their schedules
- Match clients with suitable Care Professionals, prioritizing building strong relationships
- Support with service reviews and client support plans
- Conduct staff supervisions and client quality assessments
- Facilitate client and Care Professional introductions
- Build and maintain positive relationships with clients, families, and other professionals
- Manage enquiries and participate in the on-call rota
- Excellent communication and interpersonal skills
- Strong organizational and problem-solving abilities
- Ability to work at a fast pace in a dynamic environment
- Previous experience in the care sector or related industry (desirable)
- Experience with People Planner software (advantageous)
As a Scheduler/Care Co-ordinator at Home Instead, you will have the opportunity to make a real difference in the lives of our clients. We offer a competitive salary of £24k-£26k depending on experience, and a chance to grow and develop your career within our organization.
