Outcome Coordinator

We are seeking a highly skilled and motivated individual to join our Integrated Community Discharge Team as a Care Broker Specialist. As a key member of our team, you will play a vital role in sourcing support to effectively meet the needs of people with learning disabilities, mental health conditions, physical impairments, and older people.
Key Responsibilities- Work closely with Social Workers, Community Case Workers, and Occupational Therapists to identify and source support for individuals with complex needs.
- Develop and maintain relationships with health and social care professionals, community groups, and voluntary organizations to ensure seamless support for service users.
- Assess and evaluate the needs of service users, identifying opportunities for asset-based social work and promoting independence.
- Collaborate with colleagues to develop and implement care plans, ensuring that individuals receive the support they need to achieve their goals.
- Provide excellent communication and interpersonal skills, working effectively with service users, carers, and other stakeholders to ensure that their needs are met.
- High level of organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
