Outcome Coordinator

About the Role
We are seeking a highly organized and skilled individual to join our Integrated Community Discharge Team as an Outcome Broker. The successful candidate will work closely with Social Workers, Community Case Workers, and Occupational Therapists to source support and meet the needs of individuals with learning disabilities, mental health conditions, physical impairments, and older people.
Key Responsibilities
- Collaborate with multidisciplinary teams to identify and address care and support needs
- Source and coordinate resources to meet individual needs, ensuring compliance with relevant legislation
- Promote asset-based social work and support individuals to contribute to their community
- Develop and maintain effective relationships with stakeholders, including individuals, carers, and health and social care professionals
Requirements
- High level of organizational skills and attention to detail
- Excellent communication and interpersonal skills, including written skills
- Ability to work independently and as part of a team
- Knowledge of computer programs, including spreadsheets and databases
