Outcome Coordinator

We are seeking a highly organized and skilled individual to join our Integrated Community Discharge Team as a Care Broker. The successful candidate will work closely with Social Workers, Community Case Workers, and Occupational Therapists to source support for individuals with learning disabilities, mental health conditions, physical impairments, and older people.
Key Responsibilities- Provide support to individuals with care and support needs, carers, and other health and social care professionals.
- Comply with relevant legislation, including the Care Act, the Mental Capacity Act, and the Mental Health Act.
- Promote asset-based social work and support individuals to contribute to their community.
- Work collaboratively with the team to achieve the best possible outcomes for service users.
- A high level of organizational skills.
- Sound communication skills, including written skills.
- Excellent interpersonal, negotiation, problem-solving, and analytical skills.
- A knowledge of computer programs such as spreadsheets and databases.
