Outcome Coordinator

We are seeking a highly skilled and motivated individual to join our Integrated Community Discharge Team as a Care Broker Specialist. As a key member of our team, you will play a vital role in sourcing support to effectively meet the needs of people with learning disabilities, mental health conditions, physical impairments, and older people.
Key Responsibilities- Work closely with Social Workers, Community Case Workers, and Occupational Therapists to identify and source support for individuals with complex needs.
- Develop and maintain relationships with health and social care professionals, community groups, and voluntary organizations to ensure seamless support for service users.
- Provide expert advice and guidance to individuals, carers, and families on available support options and services.
- Collaborate with the team to develop and implement care plans that promote independence, well-being, and community engagement.
- High level of organizational skills and ability to work in a fast-paced environment.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
- Sound knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and Mental Health Act.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality services.
