We are pleased to offer an exciting opportunity for a Band 6 Community Nurse Practitioner to join our Learning Disability Team in Newcastle and Gateshead. This challenging yet rewarding role allows you to work in a multidisciplinary setting, supporting individuals with complex needs who require high-quality care and intervention.

As a Community Nurse Practitioner, you will be expected to demonstrate exceptional organisational skills, the ability to prioritise workload effectively, and a strong commitment to delivering high-quality patient care.

The role involves working closely with various agencies, including social care, service providers, inpatient services, and safeguarding teams. Our team is known for its broad knowledge base and expertise in managing complex cases.

  • Responsible for assessing and managing a caseload of patients with substantial and complex needs who are at risk of hospital admission, placement breakdown, and presenting as acutely unwell.
  • Provide comprehensive assessment, risk management, safety planning, and care and treatment reviews, while offering guidance and support to colleagues.
  • Work collaboratively with patients to develop individualised care plans, ensuring regular reviews to monitor progress and adapt plans as necessary.
  • Involve carers and family members in care planning and consider their needs throughout the process.
  • Coordinate treatment planning, interventions, clinical reviews, and discharge planning to achieve optimal outcomes.
  • Deliver clinical supervision to staff as delegated by Team Manager and Clinical Leads.
  • Embed a patient-centred approach in nursing practice and ensure seamless collaboration between healthcare professionals.

Estimated salary: £44,414 - £73,362 per annum.
