Career Development Case Worker

Job Summary:
We are seeking an experienced and self-motivated Career Development Case Worker to join our Learning Disability and Sensory Work Team. This is a full-time position that requires a high degree of autonomy, effective communication, and problem-solving skills.
Key Responsibilities:
• Manage a caseload of complex cases
• Participate in liaison with other teams, both internal and external, to ensure a good quality service that is in line with our statutory responsibilities and established good working relationships
• Provide person-centred, strength-based assessments and care plans which focus on maintaining independence
• Have a detailed knowledge of adult care and its underpinning legislation
