About the Role

This is an exciting opportunity to work as a Community Psychiatric Nurse with NHS Professionals Limited, providing vital healthcare services within the community. As a Community Psychiatric Nurse, you will be responsible for assessing patients who have been referred due to presenting with mental health difficulties, identifying appropriate care pathways, and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.

Key Responsibilities

  • Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
  • Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
  • Signposting to appropriate care providers (including primary and tertiary care).
  • Raising concerns to senior staff for support and management.
  • Completing documentation and planning care for patients.
  • Presentation cases to Multi-Disciplinary Team for discussion around care pathway.
  • Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.

  • A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years.
