Family Support Worker for Children and Young People's Weight Management

We are seeking an experienced and passionate Family Support Worker to join our team at NELFT North East London Foundation Trust. The successful candidate will be responsible for delivering family support services to children and young people in our community, with a focus on promoting healthy weight and lifestyles.
Key Responsibilities- Develop and deliver family support programs to promote healthy eating and physical activity among children and young people.
- Work with families to identify and address barriers to healthy weight and lifestyles.
- Provide support and guidance to families to help them make sustainable lifestyle changes.
- Collaborate with other healthcare professionals and community organizations to provide comprehensive support to families.
- Conduct assessments and develop care plans to support families in achieving their health goals.
- Provide education and training to families on healthy eating and physical activity.
- Monitor and evaluate the effectiveness of family support programs and make recommendations for improvement.
- Degree in a relevant field such as health promotion, public health, or social work.
- Experience working with families and children in a healthcare or community setting.
- Knowledge of healthy eating and physical activity principles and practices.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to work with diverse populations and cultures.
