Job Summary

We are seeking an experienced Clinical Pharmacist to join our multidisciplinary team as a Clinical Pharmacy Specialist - Medication Optimisation. This exciting role offers the opportunity to work closely with general practice staff to provide expert guidance on prescribing and medication management.

Main Responsibilities

The successful candidate will be responsible for:

  • Providing patient-facing clinics for long-term condition management, including COPD and asthma.
  • Delivering structured medication reviews and conducting audits to identify areas for improvement.
  • Working collaboratively with the general practice team to develop and implement medicines optimisation strategies.
  • Providing education and training to primary care teams on therapeutics and medicines optimisation.
  • Maintaining accurate records and reporting on key performance indicators.

Requirements

To be considered for this role, you will need:

  • A Master's degree in pharmacy (MPharm) or equivalent qualification.
  • Completion of an 18-month CPPE pathway or equivalent experience.
  • Independent prescriber status or willingness to obtain.
  • Prior GP/clinical pharmacy experience, with specialist knowledge in multiple chronic disease areas.

Working Relationships

You will work closely with a broad range of stakeholders, including general practitioners, nurses, and other healthcare professionals, to deliver high-quality patient care and contribute to the development of our services.

Salary and Benefits

The estimated salary for this role is £60,000 - £80,000 per annum, depending on experience, with additional benefits including opportunities for professional development and a supportive working environment.
