The successful candidate will be responsible for providing high-quality, patient-centered care to individuals in the community, working closely with other healthcare professionals to ensure seamless care transitions.

Key Responsibilities:

* Provide expert clinical leadership and guidance to community nursing teams
* Develop and implement effective care plans to meet the needs of patients
* Collaborate with other healthcare professionals to ensure comprehensive care
* Conduct assessments and provide treatment to patients in their homes
* Supervise and mentor junior staff members
* Participate in quality improvement initiatives and contribute to the development of policies and procedures

Requirements:

* Diploma or Degree in Nursing
* Current NMC Registration
* Evidence of CPD maintained in a portfolio
* Supervisory and leadership skills training or equivalent experience
* Ability to perform Uroflowmetry, Bladder scanning, vaginal examination, and rectal examination

Desirable Criteria:

* MSc relevant to healthcare
* Post-registration experience in a NHS community environment
* Participation in audit; research projects; service development
