About the Role

We are seeking a highly skilled and experienced professional to join our team as a Neighbourhoods Meetings Development Manager. This is an exciting opportunity to shape and deliver the provision of co-ordinated care for vulnerable people in the London Borough of Hackney.

Key Responsibilities

  • Lead the development of multi-agency team working across the health and social care system in City and Hackney within Neighbourhoods.
  • Provide proactive support to ensure the management, development, and improvement of Multi Agency Meetings within Neighbourhoods.
  • Design, deliver, and evaluate a range of projects to improve care coordination and integration.
  • Develop and maintain effective relationships with internal and external stakeholders, including health, local authority, and voluntary sector providers.
  • Manage competing demands and prioritize tasks to meet the needs of the service.

Requirements

  • Postgraduate qualification in a relevant field or equivalent experience.
  • Experience in project management, team leadership, and communication.
  • Knowledge of relevant national policies and procedures, including the social model of disability.
  • Ability to work autonomously and as part of a team, with a strong commitment to equality and diversity.
  • Excellent problem-solving, planning, and time management skills.
