Job Summary:
We are seeking a skilled Care Coordinator to join our team in Spalding. As a key member of our staff, you will play a crucial role in coordinating the provision of care and support staff to service users, ensuring that their needs are met and exceeded.
Key Responsibilities:
- Develop and implement person-centred care plans that meet the individual needs of service users.
- Coordinate the delivery of care and support services, including on-call duty and emergency response.
- Monitor care worker and service user progress, updating care plans through quality monitoring systems and team supervision.
- Identify and report concerns about care quality or safety risks to management.
- Manage staff discipline, grievances, complaints, and absence management according to company policy.
