Job Title: Care Specialist

About the Role:

  • We are looking for an experienced NMC registered nurse to join our team as a Care Specialist in Neath/Port Talbot.
  • This role involves working closely with patients requiring enteral feeding and supporting their carers.
  • As a key member of our team, you will be responsible for managing your own caseload, working autonomously and collaborating with healthcare professionals.
  • You will provide training and clinical support to patients, carers and healthcare professionals on enteral feeding pumps and procedures.

Your Key Responsibilities:

  • Facilitate patient discharges by improving care and the overall patient experience.
  • Reduce unnecessary hospital referrals by providing effective support to those with short or long-term enteral feeding requirements.
  • Work collaboratively with local healthcare professionals to improve care and outcomes for patients.

Requirements:

  • NMC registration as a nurse.
  • Highly planned and organised approach to work.
  • Ability to work autonomously and as part of a team.

Salary: £45,000 - £55,000 per annum (dependent on experience)
