Job Description:
This role involves working as part of our dedicated Community Nursing team and the wider ICC, delivering holistic, patient-centred care in the community and complex caseload management. The successful applicant will be expected to liaise and work with the primary health care team and other Health and Social Care professionals in care provision.
Responsibilities:
Deliver high-quality care to patients in the communityLiaise with primary health care teams and other Health and Social Care professionalsManage complex caseloads and provide clinical leadershipRequirements:
To be considered for this role, you will need to have a Level 1 registered nursing qualification, evidence of continuous professional development to degree level or equivalent experience, and a Specialist qualification in Community Nursing or equivalent.
Benefits:
We offer a competitive salary of £40,000 - £60,000 per annum, depending on experience, and a range of benefits including opportunities for professional development and a supportive working environment.
