Community Health Care Professional

We are seeking a highly skilled and dedicated Senior Community Staff Nurse to join our established team at North Cumbria Integrated Care NHS Foundation Trust. This is an excellent opportunity for registered nurses looking to develop their clinical skills and expertise in caring for patients in a community setting.
The successful applicant will be responsible for delivering high-quality, patient-centered care to individuals with complex needs within the community. This includes working closely with primary healthcare teams and other health and social care professionals to provide comprehensive care services.
In this role, you will have the opportunity to demonstrate your leadership skills, operational management, and clinical expertise in managing a clinical caseload for the team. A post-registration qualification in Acute and Chronic disease management, Nurse Prescribing, and Specialist qualification in Community Nursing or equivalent are essential for this position.
Key Responsibilities:
- To deliver holistic, patient-centered care to individuals with complex needs within the community.
- To work effectively with primary healthcare teams and other health and social care professionals to provide comprehensive care services.
- To manage a clinical caseload for the team, including complex caseload management.
- To demonstrate leadership skills, operational management, and clinical expertise in managing the team's clinical caseload.
Requirements:
- A Level 1 registered nursing qualification.
- Evidence of continuous professional development to degree level or equivalent experience.
- Post Registration qualification in Acute and Chronic disease management.
- Nurse Prescribing.
- Specialist qualification in Community Nursing or equivalent.
Salary: £40,000 - £60,000 per annum (dependent on experience). An attractive salary range that reflects the importance of this role in providing exceptional care to our patients. We strive to offer salaries that are competitive within the industry and aligned with market standards.
