Neonatal Discharge Coordinator

We are seeking a highly motivated and experienced Neonatal Nurse to coordinate neonatal discharges in our unit. This post offers the opportunity to work in a level 2 Neonatal Unit (NNU) and Transitional Care Unit (TCU) based in the postnatal ward.
This is a challenging and rewarding role that requires a high level of clinical expertise, excellent communication skills, and the ability to work effectively in a multidisciplinary team.
Key Responsibilities- Follow the discharge pathway within the neonatal service and provide specialist advice and guidance to colleagues about the discharge process
- Facilitate seamless discharge of babies with complex needs from the neonatal unit (NNU) and the transitional care unit (TCU) by liaising with pertinent agencies outside the organisation
- Take responsibility as the care provider for a determined group of patients without direct supervision
- Supervise and teach junior staff and pre and post registration students whilst maintaining a safe working environment
- Work within the codes of practice, professional guidelines, and policies
- Promote the continuous improvement of clinical standards in conjunction with the application of evidence-based practice
- Utilise a variety of strategies to communicate effectively with the baby's family/carers, the multidisciplinary team, other departments within the Trust and relevant agencies outside the organisation
- Be responsible for the information governance regarding discharges for NNU/TCU and the completeness of data
- To be highly competent, knowledgeable practitioner ensuring provision of excellent evidence-based care
