Job summary

Wehave a new and exciting opportunity to appoint Housebound Healthcare Assistantsto join our dynamic and friendly operations team. Post holders will beconfident in dealing with patients, able to work independently whilst understanding the importance of proactivelyescalating concerns.

Main duties of the job

You will be providing the delivery ofservices including Phlebotomy, 24hrBP monitoring, ECGs, Warfarin Monitoring, Atrial Fibrillation Monitoring to some of the most vulnerablepatients in Hillingdon in their homes. Ideally you will already be skilled inproviding phlebotomy services and should be willing to be trained to supportthe delivery of other aspects of patient care.

You will be supported in yourrole through friendly management and administration team as well as clinicalmanagement for all patient concerns. Working in Hillingdon also means you have access toour outstanding training hub providing support and development to cliniciansworking in the area.

This will require you to loan workand travel between clinical sites and patients homes within, and up to 1 milesoutside of the borough of Hillingdon.

Job responsibilities

In accordance with the servicesneeds the post-holder will make him/her-self available to undertake a varietyof duties including Phlebotomy,ECG, AF monitoring, Post-operative dressings, PillChecks, Immunisations, Phlebotomy, Spirometry, BP checks, NHS Health Checks anddiabetic foot checks according to the agreed and provided processes andprotocols. Training will be provided in accordance with business requirements. Assessing the health needs ofpatients with chronic health needs and using your own judgment to assesswhether further input is needed. Ensuring that potentialsafeguarding concerns are reported immediately Screening patients for disease riskfactors and early signs of illness. Contributing to QOF requirementsand recording data appropriately. Contributing to PCN requirementsand recording data appropriately. In consultation with patients andin line with current practice disease management protocols, developing careplans for health related illnesses. Screening patients for healtheducation. Admitting and discharging patientsto and from the caseload and referring to other care providers as appropriate. Recording clear and contemporaneousconsultation notes to agreed standards. Collecting data for audit purposesand contributing to the practice audit programme as required. Takeresponsibility for some aspects of the clinical work within the team, forexample: coordinating aspects of governance, QOF, enhanced services, CPD andkeeping clinical protocols up to date. In general, the post holder will be expected to undertake all the normalduties and responsibilities associated with a healthcare assistant workingwithin primary care. Person Specification
