Job Title: Healthcare Support Worker

We are seeking a motivated and compassionate Healthcare Support Worker to join our team at Bourtreehill Health Centre. As a key member of our healthcare team, you will provide high-quality care and support to patients with complex health needs, including ventilation.

About the Role:

This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your skills and experience in a variety of care settings. You will work collaboratively with our experienced team of healthcare professionals to provide 24-hour care to patients in their own homes.

Key Responsibilities:
  • Provide high-quality care and support to patients with complex health needs, including ventilation.
  • Work collaboratively with our experienced team of healthcare professionals to provide 24-hour care to patients in their own homes.
  • Develop and maintain strong relationships with patients, families, and carers to ensure person-centred care.
  • Contribute to the development and implementation of individual care plans, ensuring that patients' needs are met and their goals are achieved.
  • Participate in multidisciplinary team meetings to discuss patient care and share best practices.
Requirements:
  • SVQ 2 or equivalent experience in a care setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including evenings and weekends.
