Care Coordinator

About the Role:
We are seeking a highly skilled and compassionate Case Manager to join our team at North West London Integrated Care Board. As a Case Manager, you will play a vital role in supporting individuals with learning disabilities to access the healthcare services they need.
Key Responsibilities:
- Conduct comprehensive assessments to evaluate the healthcare needs, functional abilities, and support requirements of individuals with learning disabilities.
- Develop and implement person-centered care plans in collaboration with the individual, their family, and a multidisciplinary team.
- Coordinate and facilitate the implementation of care plans, ensuring that services and supports are delivered in a coordinated and timely manner.
- Act as an advocate for individuals with learning disabilities, ensuring their needs and preferences are considered throughout the CHC process.
- Work closely with a variety of stakeholders, including healthcare professionals, social workers, care providers, and specialist learning disabilities teams.
- Identify, assess, and mitigate risks related to the care and support needs of individuals with learning disabilities.
Requirements:
- Strong communication and interpersonal skills.
- Ability to work collaboratively as part of a multidisciplinary team.
- Knowledge of the CHC process and relevant policies and procedures.
- Experience working with individuals with learning disabilities and their families.
- Ability to assess and manage risk.
