Care Coordinator

About the Role:
We are seeking a skilled and compassionate Case Manager to join our team at North West London Integrated Care Board. As a Case Manager, you will play a vital role in supporting individuals with learning disabilities to access comprehensive healthcare services and achieve their goals.
Key Responsibilities:
- Conduct comprehensive assessments to evaluate the healthcare needs, functional abilities, and support requirements of individuals with learning disabilities.
- Develop and implement person-centered care plans in collaboration with the individual, their family, and a multidisciplinary team.
- Coordinate and facilitate the implementation of care plans, ensuring timely and coordinated delivery of services and supports.
- Act as an advocate for individuals with learning disabilities, ensuring their needs and preferences are considered throughout the CHC process.
- Collaborate with healthcare professionals, social workers, care providers, and specialist learning disabilities teams to ensure effective communication, coordination, and integration of care services.
- Identify, assess, and mitigate risks to ensure the safety and well-being of individuals with learning disabilities.
- Liaise with finance colleagues to ensure accurate reflection of placement costs in the service budget.
- Accurately and timely document assessment findings, care plans, progress notes, and any changes in the individual's health or care needs.
Requirements:
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work collaboratively with a variety of stakeholders. You will also need to be able to think critically and creatively to develop innovative solutions to complex care needs.
