About the Role

We are seeking a highly skilled and compassionate Support Coordinator to join our team at Home Group. As a key member of our care team, you will play a vital role in supporting our customers who have complex dual diagnosis with severe and enduring mental health needs.

Key Responsibilities
  • Develop and implement personalized support plans to help our customers achieve their goals and aspirations.
  • Coordinate and lead a small team of Support Workers to deliver high-quality care and support services.
  • Conduct risk assessments, support planning, and regular reviews to ensure our customers receive the best possible care.
  • Collaborate with colleagues to ensure seamless delivery of services and effective communication with customers and their families.
  • Provide emotional support and guidance to our customers, promoting independence and empowerment.
Requirements
  • Experience of working in a care or support role, preferably in a mental health setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with customers and colleagues.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of our customers.
  • Enhanced DBS check and ability to use technology for creating and updating support plans.
