Job Title: Clinical Nurse Specialist for Secondary Breast Cancer

Job Summary: We are seeking a highly skilled Clinical Nurse Specialist to join our team at Hampshire Hospitals NHS Foundation Trust. The successful candidate will provide specialist nursing advice and support to patients diagnosed with secondary breast cancer, and their relatives and carers.

Main Responsibilities:

* Work as part of the multidisciplinary team within the breast care service and oncology
* Act as a key worker to the patient, coordinating their care and treatment plan
* Provide an interface between patients/carers and the multi-disciplinary team, developing nursing practices and identifying opportunities to improve the quality of patient care/experience
* Coordinate and support clinics and prioritise all referrals from MDT, leading in the assessment, planning, implementation, and evaluation of care
* Identify and refer breast patients for psychological/physio/dietetic support as required
* Exercise leadership and inspire colleagues to improve standards and quality to develop professional practice
* Identify strategies to help inform and educate the patient and family members and support them in decision making, in line with personalised care
* Participate in the development of the individual care pathway for the patient with secondary breast disease
* Undertake and facilitate nursing research and audit reporting findings to the multi-professional team
* Work with the breast care team in the implementation of local and national nursing strategy associated with breast cancer

Requirements:

* NMC registration
* First level degree, or equivalent qualification
* Evidence of continuing professional development
* Relevant clinical experience in Breast Cancer and/or Oncology
* Demonstrates knowledge of policies, practices, and procedures e.g. NHS national policies, standards requirements, and directions that relate to patient care
* Demonstrates knowledge of personalised care
* Demonstrates an ability to prioritise own workload
* Demonstrates effective communication skills
* Demonstrates ability to work independently
