About the Role

We are seeking a skilled and compassionate Field Care Supervisor to join our team at London Care Limited. As a Field Care Supervisor, you will play a vital role in ensuring the delivery of high-quality care support services to our clients.

Key Responsibilities

  • Supporting Care Workers: Provide guidance, instruction, and support to our care workers to ensure they meet the highest standards of care.
  • Assessment and Review: Conduct regular assessments and reviews of individual services to ensure our clients receive the best possible care.
  • Care Planning: Collaborate with Care Coordinators to plan effective care worker rotas that align with the specific needs of our clients and the skills of our care workers.
  • Performance Monitoring: Perform on-site observations and evaluations of care worker practices as part of routine supervision or in response to performance concerns.
  • Multi-Disciplinary Collaboration: Participate in multi-disciplinary reviews of clients' needs to provide comprehensive and coordinated care.
  • Training and Mentorship: Provide training and mentorship to new care workers to ensure their competence and confidence in delivering exceptional care.

Requirements

  • Assessment Experience: Proven experience in conducting initial risk and needs assessments for individual clients.
  • Collaborative Approach: Ability to work in partnership with clients, their families, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of comprehensive care packages.
