Job Summary

We are seeking a highly skilled and compassionate Support Worker to join our team at Home Care Direct. As a Support Worker, you will be responsible for providing high-quality care and support to our clients in their own homes. Key Responsibilities
  • Provide personal care and support to clients in their own homes, including assistance with daily living tasks, medication management, and mobility support.
  • Develop and maintain strong relationships with clients, their families, and other healthcare professionals to ensure seamless care and support.
  • Work collaboratively with the care team to ensure that clients receive comprehensive and coordinated care.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in areas such as dementia care, medication management, and communication techniques.
  • Contribute to the development and implementation of individualized care plans that meet the unique needs and goals of each client.
  • Maintain accurate and up-to-date records of client care and support, including progress notes, medication records, and other relevant documentation.
Requirements
  • Qualifications: Level 2 Diploma in Health and Social Care or equivalent.
  • Experience: Minimum 6 months of experience in a care setting, preferably in a home care environment.
  • Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, strong problem-solving and critical thinking skills, and a commitment to providing high-quality care and support.
  • Certifications: Basic Life Support (BLS) certification, First Aid certification, and any other relevant certifications.
