Job Description

We are seeking an experienced Clinical Practitioner to join our Transformation Unit at Chesterfield Royal Hospital NHS Foundation Trust. This is a full-time permanent position working 37 hours per week.

The successful candidate will provide expert enhanced clinical management for frail patients and those with multiple long-term conditions, at risk of deteriorating health.

Main Responsibilities
  • To provide high-quality care to patients and families.
  • Manage complex medical conditions.
  • Handle challenging behaviour and work with existing resources.
Requirements
  • RGN or HCPC accredited degree that can complete the additional post registration training to V300 Independent non-medical prescribers.
  • Registered with NMC (Nursing and Midwifery Council).
  • Evidence of post registration clinical professional development.

The estimated salary for this role is £46,148 - £52,809 per annum pro rata dependent upon experience.
