About the Role

We are seeking a skilled Admissions and Discharge Coordinator to join our team at Sue Ryder Leckhampton Court Hospice. The successful candidate will play a crucial role in improving and sustaining admissions and flow within the hospice, as well as across community and ward settings.

As part of this role, you will work closely with stakeholders from hospital, community, and hospice settings, including clinical staff, executives, and other key partners. You will be responsible for reviewing referrals, triaging patients, and managing discharge planning processes. Your expertise in palliative care and long-term conditions will enable you to identify service users' needs and develop effective solutions.

Key Responsibilities:

  • Provide clinical leadership, mentorship, and supervision to junior staff
  • Foster and manage relationships with internal and external stakeholders, including meetings and coordination off-site
  • Act as a key coordinator for admissions and discharges, supporting workflows to improve these areas
  • Lead, direct, support, and facilitate the identification of service users and increase reach through project work
  • E nsure all people have an active preferred place of care and death and their needs are met

Salary

The salary for this role is £32,322 - £36,428 per annum pro rata, reflecting the importance of your contribution to our team. As a valued member of Sue Ryder, you will also benefit from enhanced pay for unsociable hours, a company pension scheme, continuation of NHS pension (T&Cs apply), 27 days holiday rising to 33 with length of service plus bank holidays, NHS annual leave honoured (T&Cs apply), enhanced maternity and paternity pay, enhanced sick pay, employee networks, staff discount of 10% on new goods online, and structured induction programme and learning and development opportunities.
