About the Role

We are seeking a skilled and experienced Admissions and Discharge Coordinator to join our team at Sue Ryder. As a key member of our hospice team, you will be responsible for leading the improvement and sustainability of our admissions and flow processes across the community and ward. This will involve working closely with stakeholders in the hospital, community, and hospice to ensure seamless communication and coordination.

Key Responsibilities:

  • Review referrals and triage, working with doctors and nurses in charge to manage this independently.
  • Develop and implement effective discharge planning processes to ensure smooth transitions for patients.
  • Provide clinical leadership, mentorship, and supervision to junior staff.
  • Foster and manage good relationships with internal and external stakeholders.
  • Act as a key coordinator for admissions and discharges, supporting workflows to improve these areas.
