Job Title: Clinical Team Lead - Community Healthcare

We are seeking an experienced Clinical Team Lead to join our Early Intervention Community Team within Birmingham Community Health Care NHS Foundation Trust. This is an exciting opportunity for a highly skilled professional to make a significant impact in the field of community healthcare.

The successful candidate will be responsible for leading a team of healthcare professionals in providing high-quality care to patients with complex short-term health needs. The role requires strong leadership, mentoring, and communication skills, as well as the ability to work collaboratively with multidisciplinary teams.

A salary of £55,000-£65,000 per annum is available for this position, depending on experience.

About the Role
  • Lead a team of healthcare professionals in providing high-quality care to patients with complex short-term health needs
  • Develop and implement streamlined patient care pathways across the local Health and social care economy
  • Act as a clinical lead for the Early Intervention Community Service, advising on the care and management of patients with complex short-term health needs
  • Ensure effective communication verbally and in writing with colleagues and stakeholders, utilising electronic methods of communication within the remit of the Data Protection Act
Requirements
  • Registered AHP equivalent qualification within your profession
  • Evidence of recent Continuing Professional Development
  • ENB 997/998 Teaching & Assessing in clinical practice or similar teaching qualification
  • Educated to Degree level or equivalent qualification in nursing or relevant health field
