About the Role

We are seeking a dedicated and highly motivated Social Worker to join our Hospital Discharge team at Cheshire West and Chester Council. As a Social Worker, you will play a critical role in supporting people to develop creative and innovative solutions to meet their care and support needs.

Key Responsibilities
  • Be person-centred and reinforce a patient's choice and control.
  • Promote a patient's wellbeing throughout their journey in the acute and community setting.
  • Provide strength-based assessments and independence.
  • Complete Care Act compliant assessments.
  • Guide and support colleagues less experienced in social care processes.
  • Knowledge related legislation, policy, and frameworks.
  • Experience of safeguarding processes and procedures.
  • Complete mental capacity assessments and best interest processes for patients unable to make specific decisions.
  • Knowledge of the Deprivation of Liberty Safeguards.
  • Attend MDTs, professionals' meetings, discharge planning meetings, and best interests' meetings as may be required.
