Dementia Care Specialist

We are seeking a Registered Mental Health Nurse (Band 6) to join our multi-disciplinary team in Morecambe. This role offers the chance to work in a highly supportive and professional environment, making a real difference to patients and their families.
Key Responsibilities:- As an independent specialist practitioner, ensure effective assessment, planning, consultation, and formulation towards diagnosis within the Memory Assessment Service.
- Demonstrate specialist knowledge and understanding of strategies and interventions for dementia treatment.
- Proficiently refer patients to other agencies within the service or external services as necessary.
- Confidently identify risk factors and take appropriate actions to mitigate them.
- Facilitate clear and effective communication between GPs, LCFT services, and non-statutory services to uphold the highest standards of patient care.
- Provide mentoring and clinical supervision to staff and trainees on placement.
- Actively promote Trust values within the MAS service, aligning with NMC and HPC codes of conduct.
- Support the team manager in providing leadership within the MAS team, ensuring robust assessment, formulation, planning, and monitoring of care for patients and their carers.
