Care Coordinator Specialist

We are seeking a highly skilled and experienced Care Coordinator to join our dynamic team at Enigma Clinical Solutions. As a Care Coordinator, you will play a vital role in coordinating and managing care services, ensuring that our clients receive the highest standard of care.
Key Responsibilities:- Care Coordination: Coordinate and manage the delivery of care services to clients, develop and maintain up-to-date care plans, and schedule and allocate care workers to clients.
- Client and Stakeholder Relations: Build and maintain strong relationships with clients and their families, provide support and address any concerns or issues, and collaborate with different brokerages of various councils to facilitate the smooth provision of care services.
- Team Leadership and Support: Provide guidance and support to care workers, conduct regular performance reviews, and identify training needs.
- Administration and Compliance: Maintain accurate and up-to-date records of all client interactions, care plans, and service delivery, ensure compliance with all relevant legislation, regulations, and company policies, and prepare reports and documentation as required.
- Quality Assurance: Conduct regular quality assurance checks to ensure the highest standards of care are maintained, implement improvements and corrective actions as necessary, and ensure that care workers fulfill their hours as scheduled.
- Previous experience in a care coordination or similar role within the healthcare sector.
- Strong knowledge of care planning and coordination.
- Excellent communication and interpersonal skills.
- Ability to build and maintain strong relationships with clients, care workers, and external stakeholders.
