We are seeking a dedicated and compassionate Care Coordinator to join our team at The Copse, a rehabilitation service for men requiring a medium-term placement. As a key member of our multidisciplinary team, you will play a vital role in supporting service users with mental health conditions to achieve their recovery goals.
Key Responsibilities
- Conduct ward rounds and analyze behaviors to inform care planning
- Collaborate with colleagues to adapt care and support service users
- Update patient records and provide overall care and support
- Assist with personal hygiene and daily living tasks
