Job Description

The Transition Coordinator will work closely with curriculum and Inclusion teams to provide high-quality learning experiences for students with EHCPs. They will be responsible for establishing partnerships with local schools and external agencies to ensure a seamless transition process.

The successful candidate will have excellent communication and interpersonal skills, with the ability to develop strong relationships with young people, their families, carers, and professionals. They will be confident in their ability to navigate complex educational policies and procedures, including those related to transition and inclusion.

**Key Responsibilities:**
