About the Role

We are seeking a highly motivated and experienced Mental Health Practitioner to join our Assertive Outreach Service team. As a key member of our community-based tertiary service, you will play a vital role in providing comprehensive care packages for individuals with severe and enduring mental health problems.

Key Responsibilities
  • Assess, plan, deliver, and evaluate care for service users referred to the Assertive Outreach Service, including risk assessment and management.
  • Work autonomously, managing your time effectively and liaising closely with other agencies and disciplines.
  • Undertake the role of Care Coordinator/Lead Professional for a caseload of service users with complex mental health problems, developing therapeutic relationships and providing support and assistance.
  • Administer and monitor the effects of prescribed medication, liaising with medical staff as necessary.
  • Communicate effectively with service users and carers who may be experiencing psychotic symptoms or distress.

Requirements
  • RMN or Registered Mental Health Nurse qualification
  • Excellent assessment, risk management, organizational, and communication skills
  • Experience and knowledge of the Care Programme Approach, Community Mental Health legislation, and Hospital Admission/Discharge process
  • Ability to work autonomously and as part of a team
  • Valid driving license and access to a car
