Job Overview


The Forensic Outreach Liaison Service Transition Specialist will play a pivotal role in ensuring seamless discharge planning and efficient transfer of care between the Forensic Outreach Liaison Service (FOLS) and Community Mental Health Teams (CMHTs) in Kent.


This specialist will work closely with FOLS and CMHT teams to identify potential barriers to discharge, provide education on Community Treatment Orders (CTO), Conditional Discharge, and risk management, and facilitate effective communication among multidisciplinary teams.


Main Responsibilities



  • Identify and highlight issues affecting patient care and discharge transitions between FOLS and locality teams.
  • Collaborate with patients to support transition from secure services to community settings.
  • Communicate effectively with service users displaying challenging behaviors.
  • Develop strong relationships with stakeholders in criminal justice, statutory, and third sector organizations.
  • Educate professionals on best practices, national policies, and professional guidelines.
  • Escalate concerns regarding poor professional conduct or practice as necessary.

Salary Information


The estimated annual salary for this position ranges from £30,000 to £50,000, depending on experience and qualifications.
